Your home and work addresses: we may need them to set you up with various services in your area
- Your telephone numbers (home, work, cell) and your e-mail address: we need that to be able to schedule and verify your appointments, as well as to contact you with any follow-ups and referrals
- Some demographic information, such as your date of birth, pay grade level, and your company name: we need this information

Yes. The information you share with us will only be available to those who will be providing services to you. Your counselor/coach will discuss confidentiality with you in detail and will provide you with some written information on the subject.

ArrowAfter the initial contact will anybody check with me to see if I am making progress with the resources you have suggested?

Absolutely. A few days after you initial contact we will call you to check if you were able to connect with the specialist we referred you to and/or if you are making progress with utilizing our resources and referrals.
